Works for individual and organization NPIs. One NPI can have several Medicare enrollments (for example, one per state), and each has its own date.
How Medicare revalidation works
Every Medicare-enrolled provider and supplier has to revalidate its enrollment on a cycle: every five years for most, every three years for DMEPOS suppliers. CMS publishes each due date on its Revalidation Due Date List roughly seven months in advance, and your Medicare Administrative Contractor also sends a notice. You revalidate through PECOS or on the CMS-855 form for your enrollment type.
If the date passes without a complete revalidation, Medicare can hold payments and deactivate billing privileges. Reactivation takes a new application, and services during the gap are generally not payable. Filing early in the window, and answering any development request from the contractor within its deadline (usually 30 days), avoids that.
What does a blank or TBD date mean?
CMS has not assigned the date yet. That usually means the revalidation is more than about seven months away. CMS asks providers not to revalidate before a date is posted.
Why do I see more than one row?
Each Medicare enrollment is listed separately: different states, an individual enrollment plus reassignments to groups, or several practice types. Each one keeps its own date.
Is this the official source?
The data comes live from CMS's public dataset at data.cms.gov, which CMS updates monthly. This page is not affiliated with CMS. For your enrollment record itself, check PECOS or your Medicare contractor.
Groups: who revalidates?
The group revalidates its own enrollment on its date, and each practitioner revalidates theirs. A practitioner's revalidation also confirms their reassignments to the group, which is why the list shows reassignment counts.
