Iowa belongs to the Counseling Compact and the Social Work Licensure Compact, which decides how easily LPCs, LCSWs, LMFTs and psychologists in Des Moines, Cedar Rapids and Davenport can treat clients across state lines. It does not change payer enrollment: Iowa Medicaid, Wellmark Blue Cross and Blue Shield and Medicare each credential you on their own, whatever your license route.

Licensure Compacts in Iowa

PSYPACT for psychologists

Iowa has enacted PSYPACT, but the law does not take effect until July 1, 2027. Until then a psychologist licensed elsewhere needs a full Iowa license to treat clients located in the state, and Iowa psychologists cannot yet use APIT or TAP to reach the other 43 jurisdictions.

Counseling Compact for LPCs

Iowa has passed the Counseling Compact, but as of October 2026 it is not one of the 10 states issuing privileges, so membership does not yet let a counselor practice across the Iowa line. The Commission says members must finish readiness steps, including data sharing and testing, before going live. Next door, Minnesota is already live.

Social Work Licensure Compact for LCSWs

Iowa is one of the 33 jurisdictions listed as members of the Social Work Licensure Compact. Iowa enacted it in May 2024 (HF 2512). The compact will offer a multistate license rather than a per-state privilege, but the Commission's July 2026 update says multistate licenses are not yet available and puts completion of the data system at spring 2027, so Iowa LCSWs still need each state's license today.

Compacts Grant a License Route, Not a Network Seat

A compact privilege is permission to practice; it is not enrollment. An Iowa therapist who adds clients in another state through a compact still needs that state's Medicaid enrollment, a practice location the plans accept there, and a separate application to each commercial plan or behavioral health network in that state. CAQH (DataSpring) carries the extra license or privilege, but every payer decides on its own timeline.

Medicare for Iowa LMFTs and Counselors

Since January 1, 2024, Medicare pays marriage and family therapists and mental health counselors directly, and an Iowa LPC qualifies as a mental health counselor when the license and degree meet the federal definition. Enrollment is the CMS-855I in PECOS, reviewed for Iowa by WPS Government Health Administrators; payment is 75% of the clinical psychologist amount. LCSWs and psychologists were already eligible, so for Iowa practices this is the first time every licensed therapist on staff can be a Medicare provider.

Iowa Medicaid and Its Plans

Iowa Medicaid enrollment is filed with the Iowa Department of Health and Human Services through the Iowa Medicaid Portal Access (IMPA) system. Most members are in managed care, and each plan credentials therapists as its own application once the state enrollment is active. The largest by 2024 enrollment:

  • Wellpoint Iowa: 222,782 members, owned by Elevance Health
  • Iowa Total Care: 203,393 members, owned by Centene
  • Molina Healthcare: 177,800 members, owned by Molina

Iowa Rules for Commercial Plans

Iowa law gives commercial plans 56 days to decide, counted from the credentialing request (Iowa Code § 514F.6). Clean claims for physicians, ARNPs and PAs during the credentialing period are paid retrospectively once they are credentialed. (Iowa Code § 514F.6(1))

Questions Iowa Therapists Ask

Can an Iowa LPC see clients in another state by telehealth?

Not through the compact yet. Iowa has passed the Counseling Compact but is not issuing privileges as of October 2026, so a license in the client's state is still required. Either way, each payer in that state needs its own enrollment.

Is Iowa a PSYPACT state?

Not yet. Iowa enacted PSYPACT, effective July 1, 2027. Until then psychologists need an Iowa license to treat clients located in the state.

How does an Iowa therapist join Iowa Medicaid?

Enroll with the Iowa Department of Health and Human Services through the Iowa Medicaid Portal Access (IMPA) system. Iowa Medicaid enrolls 603,975 people across 3 managed-care plans (2024), and each plan you want, such as Wellpoint Iowa, Iowa Total Care, Molina Healthcare, credentials you separately once the state enrollment is active.

Can Iowa LMFTs and counselors bill Medicare?

Yes, since January 1, 2024. Marriage and family therapists and mental health counselors licensed in Iowa enroll on the CMS-855I through PECOS, reviewed by WPS Government Health Administrators, and Medicare pays them 75% of the clinical psychologist amount. They need 2 years or 3,000 hours of post-master's supervised clinical experience.
