Payer enrollment for dermatologists follows the same sequence as every other provider type, CAQH (DataSpring) first, then Medicare and Medicaid, then the commercial plans, but the details that stall applications are specific to the specialty, starting with the taxonomy code (207N00000X) that every payer cross-checks against NPPES. Provider Enrollment Credentialing Services enrolls dermatologists in every state at flat per-service prices and follows up on each application until approval.

Medicare for dermatologists

Dermatologists enroll on the CMS-855I. Mohs micrographic surgery is billed with CPT 17311 to 17315: 17311 and 17313 for the first stage, by anatomic area, 17312 and 17314 for each additional stage, and 17315 for each block beyond the first five in a stage. MAC policies require the same physician to act as both surgeon and pathologist; First Coast's LCD L33689 limits Mohs to MDs and DOs with verifiable Mohs training and expects the note to say why standard excision or destruction was not used, and WPS's billing article A57477 says a specimen sent to another pathologist is not a Mohs service.

Commercial and Medicaid Networks

Commercial plans credential dermatologists through the medical network with CAQH (DataSpring) verification, but which lesions qualify for Mohs is set by each plan's medical policy, such as Anthem's clinical guideline CG-SURG-90, rather than by the contract. Review those policies alongside the fee schedule before signing.

Rules Specific to Dermatologists

Appropriate use: First Coast's Mohs LCD calls the 2012 AAD/ACMS/ASDSA/ASMS appropriate use criteria a necessary starting point, and indicates Mohs only where clinical margins are uncertain and cure or reconstruction would be compromised without immediate microscopic margin control. Records must show the lesion location, size, number of stages and specimens per stage, and that treatment options were discussed.

In-office labs: CLIA (42 CFR Part 493) applies to any facility that examines human specimens to diagnose or treat disease, so a practice that processes and reads its own Mohs frozen sections or dermatopathology slides needs a CLIA certificate covering histopathology, which is high-complexity testing. A practice that sends every specimen to an outside lab does not.

What to Watch

If the practice reads its own slides, the CLIA certificate and lab director have to be in place before the first pathology claim, and the in-office lab address should match the location enrolled with each payer.

The File We Build for dermatologists

  • NPPES record checked and corrected first: legal name, taxonomy, practice address. Mismatches here are the most common silent cause of a returned application.
  • CAQH (DataSpring) profile built or repaired, every section complete, documents uploaded, attested, with each target plan authorized to view it.
  • Medicare and Medicaid applications where they apply, filed and tracked to approval with every development letter answered inside its 30-day window.
  • Commercial applications submitted the same week so the 90 to 120 day clocks overlap, with closed-panel responses appealed or re-requested on a schedule.
  • Effective dates, contract terms and fee schedules recorded, and the maintenance calendar started: CAQH (DataSpring) re-attestation every 120 days, license and malpractice expirations, Medicare revalidation.

Pricing for dermatologists

CAQH (DataSpring) $500, Medicare $500, Medicaid $500 per state, each commercial application $200, group onboarding $500 one-time, maintenance $100 per provider per quarter. Every service and price, or start the intake; about 15 minutes, with a reference number to check status any time.

Questions dermatologists Ask

How long does credentialing take for dermatologists?

Medicare, where it applies, runs 30 to 60 days and allows retroactive billing. Medicaid runs 60 to 120 days depending on the state. Commercial plans run 90 to 120 days each, in parallel, with the effective date set at contract countersignature. A complete file on day one is the only thing that shortens any of them.

Do dermatologists need CAQH (DataSpring)?

Yes for every commercial plan and most Medicaid managed-care plans. The CAQH ProView (DataSpring) profile is where plans pull licenses, education, work history and malpractice from, so it is built and attested before the first application goes in.

What does PECS charge for dermatologists?

CAQH (DataSpring) build $500, Medicare enrollment $500, Medicaid enrollment $500 per state, each commercial application $200, group onboarding $500 one-time, maintenance $100 per provider per quarter. Enrollment services bill 50% at engagement and 50% at submission. No percentage of collections.

Can PECS work with dermatologists outside New Jersey?

Yes. Every enrollment on this page is filed through national portals (PECOS, CAQH (DataSpring), Availity) or state portals that are online, so we handle dermatologists in all 50 states from Jersey City.
