Credentialing for Facility-Based and Visiting Physicians

If you operate, perform procedures or cover cases at hospitals, outpatient departments or surgery centers, and you practice on your own or in a small group, nobody on staff owns your payer enrollments. The facility credentials you for privileges, the billing company submits claims, and the enrollments in between are left to you. Our team takes that part over.

Why Facility-Based Physicians Fall Through the Cracks

  • Privileges are not enrollment. A hospital or surgery center grants privileges; every payer still needs its own enrollment that lists the places you see patients.
  • Every new facility is a payer update. Medicare expects a new practice location within 30 days, and commercial plans need the location, and sometimes the tax ID you bill under, on file before claims from it pay cleanly.
  • Gaps in plan coverage cost the most here. Patients choose the facility, not you. If the facility is in network with a plan and you are not, those cases are paid out of network, often at lower rates and after a dispute.
  • Upkeep never stops. CAQH every 120 days, Medicare revalidation every five years, reappointments at each facility, and directory updates whenever something changes. With no office manager, a lapse is usually found when a claim denies.

What We Do for You

  • Map your enrollments against your facilities. Which plans each facility takes, where you are enrolled, and where you are not.
  • Add every facility and location to Medicare, Medicaid and each commercial plan, and set up reassignments when you bill through a group or the facility.
  • Apply to the plans you are missing, starting with the ones your facilities see most.
  • Keep it current with ongoing maintenance: CAQH re-attestation, revalidations, license and document renewals, and location changes as you add or drop facilities.
  • Show you where everything stands on a status page for every application, so there is nothing to chase.

Who This Is For

Surgeons in every specialty, anesthesiologists and CRNAs, gastroenterologists, cardiologists, pain physicians, ophthalmologists, podiatrists, urologists, ENT and OB/GYN physicians, radiologists, and other physicians and advanced practice providers who work at one or more facilities without an in-house credentialing team. We work nationwide.

Frequently Asked Questions

The hospital credentialed me. Am I not already enrolled?

No. Hospital credentialing grants clinical privileges at that facility. It does not enroll you with Medicare, Medicaid or any commercial plan. Each payer has its own enrollment, and each one needs to know every location where you see patients.

Do I need to tell Medicare when I start at a new facility?

Yes. Physicians and other practitioners must report a change in practice location to Medicare within 30 days, and most other changes within 90 days (42 CFR 424.516). If you bill through a group or the facility, the reassignment has to be in place too.

What happens if I am out of network at an in-network facility?

Since the No Surprises Act, an out-of-network physician at an in-network hospital or surgery center generally cannot balance bill the patient for many services and is paid through the plan and, if needed, the federal dispute process. Being in network with the plans that facility takes avoids that.

How often does all of this need upkeep?

CAQH re-attestation every 120 days, Medicare revalidation every five years, hospital privilege reappointments typically every two years, and a payer update every time you add or leave a location. Plans must also keep directories accurate, so they ask providers to confirm their information regularly.

What does PECS charge?

Each added location or facility is $200 per payer. Each commercial plan application is $200, Medicare enrollment or reassignment is $500, and a Medicare revalidation is $250. Ongoing maintenance, which keeps CAQH, revalidations, renewals and location updates current, is $100 per provider per quarter.

Added facilities are $200 per payer, commercial plans $200 each, and ongoing maintenance $100 per provider per quarter. Flat prices, no percentage of collections.
