Credentialing for Senior Care and Facility-Based Groups

Therapy, psychiatry, primary care and rehab groups that serve assisted living and nursing facilities carry a credentialing load most practices never see: many buildings, several counties, the plans each resident happens to be on, and clinicians joining every month. Our team keeps every clinician enrolled with the right plans so visits get paid, and keeps it current as your staff and communities change.

Why Senior Care Groups Get Stuck

  • Residents bring their own plans. Medicare Advantage and Medicaid managed care plans differ by county, so a clinician who covers three communities may need a different set of plans for each.
  • Turnover never stops. Every new clinician needs CAQH, Medicare reassignment, Medicaid and each plan before their visits pay. A hire who starts before enrollment is finished produces visits nobody can bill.
  • Locations matter. Plans need to know where your clinicians see patients. Missing communities or service areas are a common reason claims deny.
  • Upkeep is constant. CAQH every 120 days, revalidations, license renewals, rosters, and payer updates whenever someone joins or leaves.

What We Do for You

  • Build your plan map from the counties of your communities: which Medicaid managed care, Medicare Advantage and commercial plans each clinician needs.
  • Set up the group once: Tax ID and group NPI linkage, group payer records and rosters.
  • Credential every clinician, including new hires through a simple intake link, with all plans worked in parallel.
  • Add your communities and service areas to each enrollment.
  • Keep it current: CAQH re-attestation, revalidations, expirations and roster changes, with a status page for every application.

Who This Is For

Behavioral health and psychotherapy groups (LCSW, LMHC, LMFT, psychologists), psychiatric practices and psychiatric nurse practitioners, primary care and geriatric practices that round in facilities, physical, occupational and speech therapy providers, podiatry, wound care and other visiting specialists, and senior living operators building an in-house clinical team. We work nationwide.

Frequently Asked Questions

Which plans do our clinicians need?

The ones your residents are enrolled in. In most states that means Medicare, the state Medicaid program, the Medicaid managed care plans in each county you serve, and the Medicare Advantage plans your residents choose, plus a few commercial plans for younger residents and family coverage. We build the list from the counties of your communities and confirm it against each plan.

Can our clinicians bill for residents in a skilled nursing stay?

Generally yes for professional services. Services by physicians, nurse practitioners, physician assistants, clinical psychologists and clinical social workers are excluded from Medicare's skilled nursing facility consolidated billing, so those clinicians bill Medicare directly. Each clinician still has to be enrolled and set up under your group.

We hire new clinicians every month. How does that work?

Each new hire gets the same building blocks: CAQH (DataSpring) build or update, Medicare reassignment to your group, Medicaid, and the plan applications for the counties they will cover. You send the hire through our intake link and we take it from there, with every application on a status page.

Do the facilities themselves need to be listed?

Plans need to know where your clinicians see patients. For groups that see residents on site, we add the communities or service area to each enrollment so claims from those addresses pay cleanly. Adding locations is $200 per payer.

What does PECS charge?

CAQH (DataSpring) $500 new or $250 update, Medicare $500, Medicaid $500 per state, and $200 for each Medicaid managed care, Medicare Advantage or commercial plan, per clinician. Group onboarding is $500 once. Ongoing maintenance is $100 per clinician per quarter. Enrollments bill 50% at engagement and 50% at submission.

Flat prices per clinician, no percentage of collections, no long contracts. Each plan application is $200; maintenance is $100 per clinician per quarter.
