Getting a provider in-network in New Jersey means three separate enrollments that share one file: Medicare through PECOS and Novitas Solutions, NJ FamilyCare through NJMMIS (Gainwell), and each commercial plan you want, starting with Horizon Blue Cross Blue Shield of New Jersey. Provider Enrollment Credentialing Services handles all three for physicians, nurse practitioners, physician assistants, therapists and group practices across the state, at flat per-service prices, followed up regularly until approval.

Typical timing from a complete application: Medicare 45 to 90 days, NJ FamilyCare 60 to 120 days, and commercial plans held to the 90-day credentialing decision deadline that New Jersey law sets (N.J.A.C. 11:24C-1.3; details below).

Medicare in New Jersey

Medicare enrollment is national, but the review happens at the Medicare Administrative Contractor for your jurisdiction. For New Jersey that is Novitas Solutions, jurisdiction JL. Individual practitioners file the CMS-855I in PECOS, groups file the 855B and each clinician reassigns benefits with the 855R, and EFT is set up with the 588. Novitas Solutions typically processes a clean application in 45 to 90 days and Medicare allows billing back to the effective date, which is why Medicare goes first: it is the one enrollment that pays for the waiting time. Development letters give you 30 days to respond, and an unanswered letter is the most common reason a New Jersey application is returned.

NJ FamilyCare

NJ FamilyCare is administered by the New Jersey Division of Medical Assistance and Health Services. Provider enrollment is filed through NJMMIS (Gainwell). Every enrollment is screened at a federal risk level: limited-risk types such as physicians, nurse practitioners and most therapists get a license and database check; moderate and high-risk types, which include many agencies and suppliers, add a site visit and in some cases fingerprint-based background checks for owners. Most New Jersey Medicaid members are in managed-care plans, and each MCO credentials and contracts separately after the state enrollment is active, so a practice that wants Medicaid patients usually needs the state enrollment plus two to four MCO applications. Revalidation is required at least every five years.

Nurse practitioners: New Jersey is a reduced practice state. See NP credentialing in New Jersey.

Therapists: New Jersey's PSYPACT, Counseling Compact and Social Work Compact status, and what each means for LPC, LCSW and LMFT enrollment, are in therapist credentialing in New Jersey.

Commercial Plans in New Jersey

The Blue Cross Blue Shield licensee for New Jersey is Horizon Blue Cross Blue Shield of New Jersey, and for most practices it is the first commercial application because it carries the largest share of employer-sponsored members in the state. Aetna, Cigna, UnitedHealthcare and Humana operate statewide and take applications through Availity or their own provider portals, verifying credentials against your CAQH (DataSpring) profile. Behavioral health clinicians apply to the plans' behavioral arms, Optum Behavioral Health, Evernorth and Carelon, rather than the medical networks. Every commercial application is $200 per provider and runs 90 to 120 days; the contract's effective date is set when the plan countersigns, not when you submit.

New Jersey Credentialing Law

New Jersey law gives commercial plans 90 days to decide, counted from a complete application for providers using CAQH, with a complete or incomplete notice within 45 days (N.J.A.C. 11:24C-1.3).

Application form: Carriers must accept the NJ Universal Physician Application and cannot require their own form instead. (N.J.A.C. 11:24C-1.3)

Credentialing inquiries must be answered within 5 business days.

The Order That Works

  1. NPI, Tax ID and, for groups, the Type 2 NPI and group W-9, all matching your legal name exactly.
  2. CAQH (DataSpring) profile built and attested, with the New Jersey license, DEA, malpractice certificate and five-year work history uploaded.
  3. Medicare through PECOS and Novitas Solutions, then NJ FamilyCare through NJMMIS (Gainwell).
  4. Horizon Blue Cross Blue Shield of New Jersey and the national plans in the same week, so the 90 to 120 day clocks run in parallel.
  5. Regular follow-up with every payer, effective dates recorded, and the maintenance calendar started for re-attestations, license expirations and revalidation.

Questions New Jersey Practices Ask

How long does credentialing take in New Jersey?

New Jersey law gives commercial plans 90 days to make a credentialing decision, counted from a complete application for providers using CAQH, with a complete or incomplete notice within 45 days (N.J.A.C. 11:24C-1.3). Contracting and loading you into the plan's systems can add time after the decision. Medicare through Novitas Solutions typically takes 45 to 90 days and allows retroactive billing to the effective date. NJ FamilyCare enrollment through NJMMIS (Gainwell) typically runs 60 to 120 days depending on risk level and whether a site visit or fingerprinting applies. Commercial plans such as Horizon Blue Cross Blue Shield of New Jersey run 90 to 120 days and set the effective date at contract countersignature.

Do I need to enroll in NJ FamilyCare before joining its managed-care plans?

Yes. New Jersey delivers most Medicaid benefits through managed-care organizations, and federal rules require every MCO network provider to be enrolled with the state program first. The state enrollment through NJMMIS (Gainwell) comes first; each MCO application follows as its own commercial-style credentialing.

Which Medicare contractor handles New Jersey?

Novitas Solutions is the Medicare Administrative Contractor for jurisdiction JL, which includes New Jersey. Applications are filed in PECOS and reviewed by Novitas Solutions; development requests and approval letters come from them.

We work with New Jersey practices the same way we work with practices next door to our Jersey City office.
