How Excellus BlueCross BlueShield Takes Applications
Excellus takes one enrollment form plus the CAQH profile. Attach licenses, a W-9 and proof of malpractice coverage of at least 1 million dollars per occurrence and 3 million aggregate; Excellus states enrollment will not be processed without them.
What to Watch With Excellus BlueCross BlueShield
