Insurance credentialing and payer enrollment in California

Getting a provider in-network in California means three separate enrollments that share one file: Medicare through PECOS and Noridian Healthcare Solutions, Medi-Cal through PAVE, the Medi-Cal Provider Application and Validation for Enrollment portal, and each commercial plan you want, starting with Blue Shield of California and Anthem Blue Cross. Medicare in California

Medicare enrollment is national, but the review happens at the Medicare Administrative Contractor for your jurisdiction. For California that is Noridian Healthcare Solutions, jurisdiction JE. Individual practitioners file the CMS-855I in PECOS, groups file the 855B and each clinician reassigns benefits with the 855R, and EFT is set up with the 588. Noridian Healthcare Solutions typically processes a clean application in 30 to 60 days and Medicare allows billing back to the effective date, which is why Medicare goes first: it is the one enrollment that pays for the waiting time. Development letters give you 30 days to respond, and an unanswered letter is the most common reason a California application is returned.

Medi-Cal

Medi-Cal is administered by the California Department of Health Care Services. Provider enrollment is filed through PAVE, the Medi-Cal Provider Application and Validation for Enrollment portal. Every enrollment is screened at a federal risk level: limited-risk types such as physicians, nurse practitioners and most therapists get a license and database check; moderate and high-risk types, which include many agencies and suppliers, add a site visit and in some cases fingerprint-based background checks for owners. Most California Medicaid members are in managed-care plans, and each MCO credentials and contracts separately after the state enrollment is active, so a practice that wants Medicaid patients usually needs the state enrollment plus two to four MCO applications. Revalidation is required at least every five years.

Commercial plans in California

The Blue Cross Blue Shield licensee for California is Blue Shield of California and Anthem Blue Cross, and for most practices it is the first commercial application because it carries the largest share of employer-sponsored members in the state. Aetna, Cigna, UnitedHealthcare and Humana operate statewide and take applications through Availity or their own provider portals, verifying credentials against your CAQH profile. Behavioral health clinicians apply to the plans' behavioral arms, Optum Behavioral Health, Evernorth and Carelon, rather than the medical networks. Every commercial application is $200 per provider and runs 90 to 120 days; the contract's effective date is set when the plan countersigns, not when you submit.

  1. NPI, Tax ID and, for groups, the Type 2 NPI and group W-9, all matching your legal name exactly.
  2. CAQH profile built and attested, with the California license, DEA, malpractice certificate and five-year work history uploaded.
