California belongs to none of the three behavioral health licensure compacts, which decides how easily LPCs, LCSWs, LMFTs and psychologists in Los Angeles, San Diego, San Jose and San Francisco can treat clients across state lines. It does not change payer enrollment: Medi-Cal, Blue Shield of California and Anthem Blue Cross and Medicare each credential you on their own, whatever your license route.

Licensure Compacts in California

PSYPACT for psychologists

California has not enacted PSYPACT, although Arizona and Nevada have. Out-of-state psychologists, including telepsychologists, need a California license to treat clients located in California, and psychologists licensed here have no compact route into the 43 participating jurisdictions.

Counseling Compact for LPCs

California has not joined the Counseling Compact, while Arizona and Nevada on its border have. An LPC licensed elsewhere who wants to see clients located in California, in person or by telehealth, needs a California license, and California counselors cannot get privileges in the 40 member jurisdictions.

Social Work Licensure Compact for LCSWs

California is not a member of the Social Work Licensure Compact. Even in the 33 member jurisdictions, multistate licenses are not yet available, so for California LCSWs the practical rule is unchanged: a license in each state where clients are located.

Compacts Grant a License Route, Not a Network Seat

A compact privilege is permission to practice; it is not enrollment. A California therapist who adds clients in another state through a compact still needs that state's Medicaid enrollment, a practice location the plans accept there, and a separate application to each commercial plan or behavioral health network in that state. CAQH (DataSpring) carries the extra license or privilege, but every payer decides on its own timeline.

Medicare for California LMFTs and Counselors

Since January 1, 2024, Medicare pays marriage and family therapists and mental health counselors directly, and a California LPC qualifies as a mental health counselor when the license and degree meet the federal definition. Enrollment is the CMS-855I in PECOS, reviewed for California by Noridian Healthcare Solutions; payment is 75% of the clinical psychologist amount. LCSWs and psychologists were already eligible, so for California practices this is the first time every licensed therapist on staff can be a Medicare provider.

Medi-Cal and Its Plans

Medi-Cal enrollment is filed with the California Department of Health Care Services through PAVE, the Medi-Cal Provider Application and Validation for Enrollment portal. Most members are in managed care, and each plan credentials therapists as its own application once the state enrollment is active. The largest by 2024 enrollment:

  • L.A. Care Health Plan: 2,359,500 members
  • Health Net Community Solutions: 1,582,968 members, owned by Centene
  • Inland Empire Health Plan: 1,492,702 members

See all 23 plans; confirm with each one whether it credentials behavioral health directly or through a vendor.

California Rules for Commercial Plans

California law gives commercial plans 90 days to decide, counted from a completed application including all third-party verifications, which plans must meet by January 1, 2027 under AB 1041 (2025); mental health and substance use providers already had 60 days (Cal. Health & Safety Code § 1374.198; Cal. Ins. Code § 10144.565). Plans and insurers must use the CAQH credentialing form starting January 1, 2028. (Cal. Health & Safety Code § 1380.2; Cal. Ins. Code § 10110.9) If the plan misses the deadline, the provider is provisionally approved for 120 days, but only if the plan credentialed them within the past five years and there is no licensing discipline, adverse action or malpractice payment report. (Cal. Health & Safety Code § 1374.198)

Questions California Therapists Ask

Can a California LPC see clients in another state by telehealth?

Only with a license from the state where the client is located. California is not in the Counseling Compact, so there is no privilege-to-practice route from here.

Is California a PSYPACT state?

No. California has not enacted PSYPACT, so telepsychology with clients located in California requires a California license.

How does a California therapist join Medi-Cal?

Enroll with the California Department of Health Care Services through PAVE, the Medi-Cal Provider Application and Validation for Enrollment portal. Medi-Cal enrolls 14,305,060 people across 23 managed-care plans (2024), and each plan you want, such as L.A. Care Health Plan, Health Net Community Solutions, Inland Empire Health Plan, credentials you separately once the state enrollment is active.

Can California LMFTs and counselors bill Medicare?

Yes, since January 1, 2024. Marriage and family therapists and mental health counselors licensed in California enroll on the CMS-855I through PECOS, reviewed by Noridian Healthcare Solutions, and Medicare pays them 75% of the clinical psychologist amount. They need 2 years or 3,000 hours of post-master's supervised clinical experience.
